Benefit Plan
Since Fall 2005, graduate students have been automatically enrolled in an extended medical/dental plan and fees have been charged along with tuition. Students who already have extended medical/dental plans through their employment have a ONE-TIME OPPORTUNITY TO OPT OUT of the plan at the beginning of their program.
- For the Fall semester this option is available until September 30.
- For Spring semester this option is available until January 30.
- For Summer semester this option is available until May 30.
After this date, all students who have not opted out will be required to pay the benefit plan fees every semester and will not have an opportunity to opt out again for ONE FULL YEAR (i.e., at the start of the second year of their program).
This benefit plan was initiated and is run by the SFU Student Society. Field Programs has no connection with this plan and all enquiries should be directed to the Student Society directly. For more information on this plan or opting out, please go to the SFS Benefits page.

Change of Name
If you change your name from the one on your original application to SFU, official evidence of your name change must be submitted to the University. Send proof of your name change to the Dean of Graduate Studies Office, Maggie Benston Building, Simon Fraser University, 8888 University Drive,

Grades
Grades are available at the end of the last month of each semester, i.e. at the end of December (for fall), April (for spring) and August (for summer). You may access your grades on goSFU. Following the provincial guidelines on confidentiality, the Field Programs Office staff cannot release grades by telephone.
All Field Programs Graduate Diploma courses are graded Satisfactory/Unsatisfactory rather than with a specific letter grade. Satisfactory is the highest that can be achieved.
All Field Programs undergraduate courses are graded Pass/Withdraw.
The Master of Education (Educational Practice) is a graded program.

Library Access
You may access SFU library services via the internet (http://www.lib.sfu.ca/) or by visiting a library at one of the SFU campuses. In order to do this, you must first obtain a library card or a library barcode number.
Library subscribers have access to library catalogues and also an ever-increasing range of electronic journals, documents and e-books.
Obtaining a Library Card
Library/Photo ID cards are issued by the Student Services offices at each campus. You are eligible for a card once you have been admitted to the University and your $100 fee for the semester has been paid. Please contact the SFU Student Services office at the campus you plan to visit for their hours of operation. Contact information can be found at http://students.sfu.ca/.
If you are in a program based outside the Lower Mainland and would like a library card, the Student Services area will prepare one and have it sent to you on request. However, you will need to supply a recent passport-sized photograph and have it signed by a Field Programs person who can authenticate your photo. If there are a group of students from the same cohort who require cards, they should all be sent together to Field Programs.

Obtaining your Library Barcode
You may obtain your library barcode via the SFU website (Guide in PDF format available).
Using the SFU Library Telebook Service
Faculty of Education students are eligible to use the SFU Library Telebook service. This allows Education students to request books and articles to be sent to them at the address on file at the university. Books and other materials must be returned in the postage pre-paid envelope included in the shipment, articles may be kept.

Computing ID
To access any on-line services and to receive important information via your SFU e-mail account you will need an SFU computing ID.
You may activate your SFU computing ID yourself via the SFU website as soon as you have been admitted as a student and have paid your deposits. (Guide in PDF format available)
You must ensure you remember both your computing ID and password as you will need them throughout your program and even after your graduation semester, (i.e., to order a gown for your convocation ceremony). Your computing ID will remain active for three semesters after you complete your coursework.
Please Note:
The Field Programs office, the University and the Student Society will use ONLY your SFU e-mail account to communicate important information to you, so you should check this account for messages regularly, or have messages forwarded to another e-mail account you use regularly.

Student Accounts
You can use your computing ID and password to access, view and print a Statement of Account showing your fee transactions each semester. Like your grades, this information is confidential and cannot be mailed or faxed to you.
If you plan to claim back tuition from your district or another source, the Statement of Account will provide proof of payment.

Tax Receipts
You can also use your computing ID and password to access, view and print your T2202A official tax receipt showing fees paid for the previous calendar year. This is usually posted each February. Please note that the Graduate Diploma Program is considered a part-time professional program. However, your T2202A will show a full-time designation for any semester where you have paid for more than six credits This is an SFU designation of full-time. You should consult with a tax accountant or CCRA directly about whether this meets CCRA's designation of full-time for tax purposes.
